* If you are designing your own degree, you must have an approved Baccalaureate Contract before submitting this form.

** For Summer 2008, any students with an RODP major (Interdisciplinary Studies, Information Technology & online Organizational Leadership) must request a permit by 4:30 p.m., Wednesday, May 28. The first day for R50 courses is Monday, June 2.

For all other majors, the deadline to request a permit for Summer 2008, is 4:30 p.m., Wednesday, June 4.

The deadline to request a permit for Fall 2008 is 4:30 p.m., Wednesday, August 20, 2008.

Requests submitted after the deadline will not be processed.
